NOTE:  You must complete the survey in one sitting.  You cannot partially complete the survey and come back.  Your data will not be saved.  You must hit submit at the end of the second section to complete your survey. The survey will close on March 2nd. 

The link will take you to a landing page where you will first input your percentages.  Below is a screen grab of the first section of the impact survey.

After you have filled out the data section of the survey, click next.  You’ll be taken to the second and final section of the survey where you will input your impact stories and upload photos.  You can input up to 10 stories and photos.

Once you have uploaded your stories and photos, hit submit and your survey will be complete.

If you do not hit submit, your survey will not be complete and your data, stories and photos will not be saved.  Please complete the survey in one sitting.  Thank you!
